On the part where they want a signature of a witness do they want a notery or can it be anyone ?

Paul, I did not see any requirement of it needing to be notarized, so I just had a neighbor witness my signature and sent it in........Tom

Thanks Tom , I went up to the range yesterday and one of the guys called the pres. Of the club while I was there ,come to find out its just a standard form that there lawyer put together and I was told not to worrie about it as long as my signature was on it along with my name printed out and dated .
Thanks for getting back to me on this I sent a pm to some members and they didn't even bother to reply back, at least you gave me some help and that's much appreciated !
